| Feel-good Friday The word on my report was degeneration, and I read it the way you would read a fire alarm. Disc degeneration at several levels. Nobody sat me down and explained it… so I looked it up at eleven at night, which I do not recommend to anybody. So here is what I wish I had found first. A team at the Mayo Clinic pulled together 33 studies covering 3,110 people. What all those people had in common is that not one of them had any back pain. [Yes](mailto:hello@thehealthbriefdaily.com?subject=Scan%20fright%3A%20yes&body=Yes%2C%20it%20did.) [A little](mailto:hello@thehealthbriefdaily.com?subject=Scan%20fright%3A%20little&body=A%20little.) [No](mailto:hello@thehealthbriefdaily.com?subject=Scan%20fright%3A%20no&body=No.) [Never had one](mailto:hello@thehealthbriefdaily.com?subject=Scan%20fright%3A%20none&body=I%20have%20never%20had%20one.) What turned up in people who felt fine They scanned them all and counted what showed. At twenty years old, 37 in every 100 already had disc degeneration. Thirty in 100 had a disc bulge. By eighty it was 96 in 100 for degeneration, and 84 in 100 for bulges. The other findings climbed too. Disc protrusion went from 29 in 100 at twenty to 43 in 100 at eighty. Small tears in the disc wall, from 19 to 29. Every one of those numbers comes from people with no symptoms at all. Not one of those people had any back pain at the time. So what is the word worth On its own, very little. The authors said it plainly enough. Many of these findings are likely part of normal aging and unassociated with pain. Degeneration on a spine report means roughly what gray hair means on a head. It tells you time has passed. And it runs the other way too. Guidelines advise against routine scans for ordinary back pain, because no evidence exists that scanning improves how people get on. Now the honest half None of that makes scans useless… and none of it makes your pain imaginary. A scan is how the rare and serious things get found, which is exactly why the short list below matters. If one of those fits you, get the picture taken. And a finding on a film can genuinely be the source of somebody's pain. The point is not that the picture lies. The point is narrower. A picture read without the person in front of you cannot tell you where pain comes from… because the very same picture turns up in people who feel nothing at all. What the evidence does say Spine degeneration on imaging is extremely common in people with no pain whatever, and it climbs steeply with age. 37 in 100 at twenty, 96 in 100 at eighty. What it does not say It does not say your scan is meaningless or your pain invented. It says a picture, read on its own, cannot tell you where the pain is coming from. That was the week. Monday, ten slow knee rolls before you stand up. Tuesday, why most back pain never gets a name. Wednesday, five walks a week and 96 extra days. Thursday, the lifting posters that do not work. Anyway, the report on my desk still says degeneration. I have stopped reading it as a sentence. It is a photograph of a sixty-seven-year-old spine. That is all it has ever been. When to stop and call your doctor A scan finding is not a diagnosis on its own, and this issue is not a reason to cancel one. Go to a hospital now if you lose bladder or bowel control. Same if you go numb around the saddle area. A leg weakening by the day counts too. Call your doctor promptly for back pain after a fall, or with fever or weight loss you cannot explain. Same for a first severe episode over 50, for night pain that will not settle, and for any history of cancer. Those are the times a picture earns its keep. Quick poll · tap to answer Where does it hurt most? Tap the spot that troubles you most. It sets where we start on Monday. Just press send.
