# The bumps on my fingers are not a warning

| How it works There is a hard lump on the middle knuckle of my right index finger. It has been there about a year. I noticed it in the spring, doing nothing in particular, and it worried me more than my thumb ever has. A pain you can argue with… a lump is a fact. So I looked it up, the way I look everything up now, and found it already had a name. Two doctors got there long before me, and each one got a knuckle. Yesterday we did the move. Today, the map. The one nearest the nail, and the one in the middle. A lump on the hinge nearest the nail is called a Heberden node. A lump on the middle hinge is a Bouchard node. Both are named for the physicians who first wrote them up, and both are the same thing under the skin. That thing is bone. Not a growth, not a cyst… not a lump of gristle. When cartilage in a joint thins out, the body lays down extra bone at the rim, and those little spurs are what you feel through the skin. Doctors call them osteophytes. So a bump is a record of wear. Like a callus records work. It is your body's answer to a joint that has lost some of its cushion. The part that surprised me Here is what I did not expect. Bouchard nodes are the less common of the two, and neither one reliably hurts. DermNet puts it plainly: these bumps may become inflamed and painful, but they are often painless and go unnoticed. So the lump is not the pain, and the size of the lump is not the size of the problem. Mine does not hurt at all. My thumb, which has no lump worth speaking of, is the one that quit on the jam jar. So you cannot read your hand by looking at it. Nor can anyone else. What the pattern does tell you The useful signal is not the bump. It is which joints are involved, and how the morning goes. The ordinary wear pattern The hinge nearest the nail, the middle hinge, and the base of the thumb. Bumpy, sore on gripping, and loosest after the hand has been used a while. Often worse in one hand than the other. A pattern that belongs to a doctor The big knuckles where the fingers meet the hand, the wrists, and both hands matching each other, with morning stiffness that runs past an hour. That is a different sort of arthritis, driven by the immune system, and it wants naming early. By the way, I am not handing you a diagnosis there. I am handing you the two questions a doctor will ask you anyway: which joints, and how long in the morning. And there is one more thing worth saying about those bumps, because I know what the worry underneath it is. A lump on a finger is not a countdown to a useless hand. It is a joint that has adapted. Not prettily… and not reversibly. But adapting is what a working body does, and yours is still doing it. So the two minutes we did yesterday are not about the lumps. They are about keeping the range you still have. When to stop and call your doctor Bumps that sit quietly are the ordinary kind. See your doctor if a finger joint turns hot, red and swollen over days, if the big knuckles and both wrists are sore together, if morning stiffness runs past an hour, or if a lump grows quickly or breaks the skin.
