| How it works I spent a year waiting for somebody to tell me what was wrong with my back. Nobody ever did. Scans. Specialists. One cortisone shot. And at the end of all of it, no name for the thing. I took that as a failure of medicine, and for a long while it left me feeling like a man being fobbed off. It took me far too long to work out that the absence of a name is the ordinary result. Not the exception. A quick question · tap to answer How long have you had back trouble? Tap one. A ready-to-send reply opens, just press send. [Under three months](mailto:hello@thehealthbriefdaily.com?subject=Back%20length%3A%20%3C3m&body=Under%20three%20months.) [About a year](mailto:hello@thehealthbriefdaily.com?subject=Back%20length%3A%20year&body=About%20a%20year.) [Several years](mailto:hello@thehealthbriefdaily.com?subject=Back%20length%3A%20years&body=Several%20years.) [On and off forever](mailto:hello@thehealthbriefdaily.com?subject=Back%20length%3A%20forever&body=On%20and%20off%20forever.) The number that settled it for me In one large look at people arriving at primary care with back pain, a specific cause was found in about one in a hundred. Fracture, infection, cancer, inflammatory disease. Those are the ones nobody must miss… and they are rare. For very nearly everybody else, there is no single structure you can point at and blame. Doctors call that non-specific low back pain. It sounds like a shrug. It is closer to a finding. How big a thing this is Around 540 million people have back pain at any one moment. That is one person in fourteen, alive right now, with a sore back. It is not a condition of the unlucky. It is closer to a condition of having a spine and using it for sixty years. It is the number one cause of disability on earth. Not heart trouble. Not cancer. Backs. 60.1 million years of life lived with disability in 2015, and that figure was up 54% since 1990. What usually happens next Most episodes improve a good deal inside six weeks. That is the half people quote at you. Here is the half they leave off. Two thirds of people still report some pain at three months… and about the same share still report some at a year. And roughly a third of the people who do recover have another episode within twelve months. So the honest shape of it is not a clean break followed by a cure. It is a thing that flares, settles most of the way, and comes back. Why that changed how I hold it If you expect a cure, every flare is a failure and every good month is borrowed time. If you expect something that comes and goes, a flare is weather. You have been here before, and you know roughly how long it lasts. That is not positive thinking, which I have no patience for. It is having the right picture… and the right picture happens to be the less frightening one. Anyway, the practical end of it is this. A flare is not evidence that you have failed at your exercises, and it is not evidence that something has finally torn. It is evidence that you have a back, and that you have had it a while. What non-specific actually means No single structure has been identified as the source. In primary care a specific cause turns up in around one case in a hundred. The rest is ordinary back pain, and ordinary is not the same as nothing. The shape of a typical episode Most improve substantially inside six weeks. Around two thirds still report some pain at three months and at a year. About a third have another episode within twelve months. None of which means you should not be checked. The rare causes are rare, not absent. The list below is short and worth knowing by heart. When to stop and call your doctor Go to a hospital now if you lose control of your bladder or bowel, if you go numb around the saddle area between your legs, or if a leg is getting weaker day by day. Call your doctor promptly for back pain after a fall, back pain with fever or weight loss you cannot explain, a first severe episode after the age of 50, night pain that will not settle, or any history of cancer.
