- Description: Fourteen trials say the same thing twice, in opposite directions.

| Live well I gave up a weekend to taking up rugs. The evidence says it may have done nothing at all. This is the honest one of the week, and it surprised me enough that I read it twice. Clearing trip hazards at home does cut falls. That part is not in doubt. But whether it does a thing for you turns on one question… and it is not how tidy your house is. A quick question · tap to answer Do you get up in the night? Tap one. A ready-to-send reply opens, just press send. [Most nights](mailto:hello@thehealthbriefdaily.com?subject=Up%20at%20night%3A%20most&body=Most%20nights.) [Sometimes](mailto:hello@thehealthbriefdaily.com?subject=Up%20at%20night%3A%20some&body=Sometimes.) [No](mailto:hello@thehealthbriefdaily.com?subject=Up%20at%20night%3A%20no&body=No.) The review Fourteen trials, 5,830 people, all of them living at home. Across everybody together, cutting hazards in the home reduced the rate of falls by 26%. Fair enough, and that is the figure that gets quoted. Now split the people up. The split Among people already at higher risk, meaning a recent fall or a recent stay in hospital, the rate of falls fell by 38%. The reviewers graded that high certainty. Among people not picked out as at risk, the rate did not fall at all. The figure came out at 1.05, with a range sitting squarely across no effect. Also graded high certainty. Not a small effect. Not a hint of one. Nothing. That second finding is the one to sit with. It is not that the evidence is thin for the lower risk group. The evidence is good, and it says nothing happened. So what do you do with it If you have fallen in the past year, or you have been in hospital, the home work is one of the best things on the table. 38% is a large cut for an afternoon and a screwdriver. If you have not, and you are steady on your feet, taking up your rugs is not the lever. Monday and Wednesday are the lever. None of which is an argument for leaving a cord across the hall. It is an argument about where to spend the afternoon. And I think I know why it splits that way. A hazard is only a hazard if you are close enough to the edge to be caught by it. A steady person walks over a rug a thousand times and nothing happens. Take away their margin, by a bad year or a spell in hospital, and the same rug starts collecting people. The finding I did not expect Eyesight. You would think better vision would cut falls, and the same review found that vision interventions may make little or no difference to the rate of falls. Low certainty on that one, so hold it loosely. But get your eyes tested because they are your eyes… not as a falls plan. If you are in the group this helps Start where falls actually happen. The route from your bed to the bathroom, in the dark, half awake. One route, and one night light. That is the whole first job. A light you can reach without getting up first. Nothing at all on the floor along that route, and that includes the rug you are fond of. Then the stairs. A rail you can hold the whole way down… and a switch at both ends of it. Then the bathroom. A grab rail bolted into the wall. Not a suction cup, which comes off in your hand at the worst possible moment. Anyway, none of that needs a contractor or a weekend. A night light, a screwdriver and a hard look at one route through the house. Who this helps People who have fallen in the past year or been in hospital lately. For them, cutting home hazards reduced the rate of falls by 38%, on high certainty evidence. Who it does not People not at raised risk. For them the same work showed no reduction at all, also on high certainty evidence. Spend the afternoon on the balance work instead. My rugs are still up and I am not putting them back. Though I will admit I did it for the tidiness and then called it prevention. Carol has a better memory than me about which of those it was. When to stop and call your doctor Getting up in the night is when a lot of this happens, so mind the reasons for it. Tell your doctor if you are up several times a night, if you feel faint or woozy on standing, or if a new medicine arrived around the same time your steadiness left. And if you have had a fall, say so out loud at your next appointment. It changes what is worth doing, as today shows. Quick poll · tap to answer Is there a light you can reach from the bed? Tap one and just press send. [Yes](mailto:hello@thehealthbriefdaily.com?subject=Bed%20light%3A%20yes&body=Yes.) [No](mailto:hello@thehealthbriefdaily.com?subject=Bed%20light%3A%20no&body=No.) [I use my phone](mailto:hello@thehealthbriefdaily.com?subject=Bed%20light%3A%20phone&body=I%20use%20my%20phone.) I read every reply. Here is to the bedroom to bathroom route,**Walt** |
